If an individual requests the disclosure of, correction to, deletion of, or the suspension of use of the Personal Information held and managed by the Company (hereinafter referred to as “Disclosure Requests”), we will confirm that the person making the request is the said individual and handle the required process in a reasonable manner. A portion of said Personal Information can also be viewed by the individual directly from this website.
Be aware that we may not be able to provide the individual’s desired services after such usage suspension or deletion of personal information.

Documents to Submit
To confirm the identity of the individual placing the request, please include a copy of one of the documents listed below.
Personal identification documents
1. Driver's license
2. Passport
3. Basic resident registration card
4. Certificate of residence
5. Family registry
6. Health insurance or national health insurance ID
7. Residence card
If the requestor is a minor, a legally-appointed adult ward, or a proxy appointed by the individual to make a request after the disclosure, please also include the documents listed below.
Legal Representatives: Please submit documents A and B.
If you are the guardian of the individual: A copy of the individual’s family registry
If you are an adult ward: The registration certificate showing that you are an adult ward
B. Documents that confirm that you are a legal representative
Also include a personal identification document as listed above (a driver’s license, passport, health insurance ID, etc.)
Proxies: Please submit documents A, B, and C.
If you are the guardian of the individual: A copy of the individual’s family registry
B. One registered seal certificate
C. One document proving that you are the said proxy (a personal identification document as listed above)
Fees and Fee Collection
Our prescribed submission method will incur a fee of 500 yen per submission.
Please include a 500-yen stamp together with your documents.
Purpose of Use of Personal Information Acquired Through Disclosure Requests
Personal Information acquired through Disclosure Requests will only be handled within the necessary scope of the Disclosure Request. Once a Disclosure Request has been handled, the submitted documents will be kept on file for one year and then disposed of in a secure manner.
Nondisclosure
We will not disclose information in the cases below. If we decide not to disclose something, we will share about said decision and the reason.
When we cannot confirm the identity of the individual, such as when the information on the application form does not match that noted in the personal identification documents.
When we cannot confirm that a proxy has the right to represent.
When a prescribed form is not filled out correctly.
When the rights or interests of the individual or a third party, including their life, wellbeing, and assets, may be harmed.
When there would be a violation of any other laws or regulations.
